Who Should Fill EPF Form 11

When a person joins a new organisation that has been registered under the EPF Scheme of 1952, they are required to complete EPF Form 11. However, since this form includes all the information pertaining to the employee's contribution, current members must also complete it if they change jobs. Moreover, you can transfer your PF account using this form.

Important Highlights of EPF Form 11

Here are some important points to note about EPF Form 11:

  1. Form 11 is a PF declaration that contains the basic EPF information of new EPF members. In case of existing members, it helps in transferring the information to a new account automatically.
  2. An individual earning a monthly salary of more than Rs.15,000 and working in a company with more than 20 employees is eligible to fill this form. 
  3. An individual needs to fill EPF Form 11 when they join a new organisation.

Previously, employees used to transfer to a new EPF account by filling out Form 13. However, since Form 11 was introduced, an automatic transfer request has been a part of this form.

Contents of Form 11

The revised structure of EPF Form 11 requires employees to fill in the following information.

  1. Name of employee
  2. Date of Birth of employee
  3. Father's/Husband's name
  4. Gender
  5. Mobile number
  6. Email ID
  7. Relationship of employee with Employees' Provident Fund (EPF) Scheme and EPS (Employees' Pension scheme) whether he/she was a member of these schemes
  8. Previous employment details, including UAN, last working day and scheme certificate number
  1. Educational credentials
  2. Marital status
  3. KYC details including bank account number, Driving license/election card, etc.
  4. If an employee is a foreigner, then passport number is required

An employer is also expected to provide a declaration stating certain facts regarding the information provided by an employee. This declaration contains the following details.

  1. Date of employee joining work
  2. PF ID number assigned to employee
  3. UAN number of employee
  4. Verification of KYC credentials

How to Fill EPF Form 11

In EPF Form 11, you need to complete the following:

Details such as your name, father/spouse name, date of birth, etc.

Information about the previous employer and EPF and EPS participation:

  1. Whether the employee is currently enrolled in the 1952 Employee Provident Fund Scheme
  2. If the employee currently participates in the Employee's Pension Scheme, 1995
  3. If the employee selected 'YES' when asked if they would like to participate in either of the two schemes, the following extra information must be provided:

1) Universal Account Number

2) Previous Provident Fund Account Number

3) Exit date from the previous job, given in the dd/mm/yyyy format

4) Pension Payment Order (PPO) No. if issued

5) Scheme Certificate No. (if issued)

The following information must be given in the case of foreign employees:

  1. Country of Origin
  2. Passport Number
  3. Passport’s validity

KYC Information:

This form must also have self-attested copies of the documents mentioned below:

  1. IFSC 
  2. Bank account
  3. PAN
  4. Aadhaar number

Employee's Undertaking:

  1. After reading the declaration points included in the form, sign the undertaking, making sure to write the date and location of the signature on the left side.

Declaration by the Present Employer:

The current employer must fill out the relevant details and take the necessary steps as outlined below before signing and sealing the document. The organisation must also submit a declaration with information an employee provided. The following information is included in this declaration:

  1. The employee's date of joining
  2. Employee’s UAN
  3. The employee's Member ID or PF ID number
  4. Verification of KYC details

Submission of EPF Form 11:

Once Form 11 is filed by the employee, it must be submitted to the employer. The form will be signed and stamped by the employer. The form will then be sent by the employer to the local EPF office. If you have a UAN, you can complete the transfer process online with ease.

Important EPF Points for the Employer

Here are the responsibilities of the employer with respect to EPF:

  1. Make sure that every new employee completes the declaration form in a month and uploads the data to the UAN portal no later than 25 days after the end of each month.
  2. Within 15 days of receiving the UAN, the employer must provide the EPFO-generated UAN details to each current member and obtain their acknowledgement.
  3. After receiving the UAN, the employer should ensure that members provide their seed KYC information (PAN, Aadhaar card, and bank account details) within a month.
  4. The employer must submit the Aadhaar Acknowledgment Slip within one month of receiving the UAN if the member does not possess an Aadhaar card.
  5. Upon obtaining the employee's Aadhaar information, the employer must upload the information on the UAN Portal within 15 days of that information being received.
  6. Before submitting claim forms to the EPFO, the employer must ensure that they are thoroughly reviewed. The employer needs to verify that everything is completed, including linking the UAN to all pertinent KYC details.

Important EPF Points for International Employees

Two categories of individuals are referred to as 'international workers:

  1. An Indian individual who currently works or has worked in another country with which India has an agreement, commonly referred to as the Social Security Agreement (SSA).
  2. Any non-Indian employee of a company covered by the Employees' Provident Fund and Miscellaneous Provisions Act who is employed in India.

The Employees' Provident Fund Scheme was previously ineligible for foreign employees working in India. However, all eligible foreign workers, or non-excluded members, must now enrol in and make contributions to the EPF Scheme.
