How to Fill out the RTO Form 1?

RTO Form 1 is divided into two sections:

Personal Information

  1. Points 1 and 2: Enter your full name and your father's name (as per the identity proof submitted). Married women can mention their husband's name.
  2. Point 3: Provide your permanent address as mentioned in the address proof.
  3. Point 4: If you are at a temporary address, mention it here.
  4. Point 5: Enter your date of birth and your current age.
  5. Point 6: Mention two identification marks (e.g., birthmarks or visible scars on the face, arms, or legs).

Medical Declaration

  1. Point a: Declare if you have a history of epilepsy. Mark 'no' if you have never experienced seizures or unusual sensations.
  2. Point b: State if you can read number plates from 25 metres in daylight and if you have full vision in both eyes. Mark 'yes' if you have good eyesight.
  3. Point c: Indicate if you have any motor disabilities or muscular problems. Mark 'no' if you do not have these issues.
  4. Points d and e: Confirm whether you can distinguish pigmentary colours and whether you are not colourblind. Mark 'no' if you can differentiate between red and green colours and have no colour blindness.
  5. Point f: Declare if you have the auditory ability to drive a light vehicle without hearing aids. Mark 'yes' if you can.
  6. Final Declaration: State if you have any other ailments or disabilities that could endanger passengers or the public. Mark 'yes' if you are in good health.

How to Download Self Declaration Form 1?

Follow the procedure mentioned below to download the form:

Step 1: Visit https://parivahan.gov.in/parivahan/

Step 2: Choose ‘Driving License Related Services’. The option can be found under ‘Online Services’.

Step 3: Choose the state you want to apply from.

Step 4: Click on the 'Others' tab present at the top-right of the page.

Step 5: From the drop-down menu, select the 'Print Application Form' option.

Step 6: Fill in the 'Application number' and 'Date of Birth'.

Step 7: Download 'Form1'.

Given below are the points you must consider when filling out Form 1: 

  1. If you have affirmed 'yes' to all statements concerning physical abilities, you must furnish additional evidence demonstrating your capability to operate a vehicle safely without endangering other commuters. Answering 'no' to points (b) and (d) also requires a statement affirming your competence to handle a vehicle.
  2. In addition to Form 1 for obtaining a driver's license, you must also submit a medical certificate in Form 1A. It's important to note the distinction between Form 1 and Form 1A.
  3. Form 1 is a self-declaration/application form required for DL applicants aged between 17 and 40, while DL candidates aged 40 and above must complete Form 1A. In India, individuals over the age of 40 must submit Form 1A.
