Online Application for Name Change in Passport

Follow these steps for the online application process: 

1. Access the official Passport Seva portal by clicking on the link 
2. Register and log in with your credentials. 
3. Choose ‘Apply for Fresh Passport/Re-issue of Passport’ from the options. 
4. Select alternative 1 for the online application. 
5. Opt for ‘Re-issue of the passport’ and specify the details you want to change. 
6. Fill in the required information related to the name change. 
7. Agree to the terms and conditions, save your details, and proceed to the next step. 
8. Choose the documents as proof of address and birth. 
9. Print the application form, make the payment, and save the receipt. 
10. Book an appointment slot based on availability. 
11. Upload scanned copies of the documents and download the final receipt. 

After completing these steps, attend the scheduled appointment at the passport office with the application receipt and relevant documents. The authorities will verify the information, and the updated passport will be sent to your home address. 

Offline Application for Name Change in Passport 

Follow these steps for the offline application process: 

1. Create an affidavit detailing the applicant's information, including the old and new names, along with the reason for the change. 
2. After completion, the Gazette of India will publish the new name. 
3. Place two advertisements about the name change in separate newspapers.
